I always try to work out the angles with the photogs beforehand.  Also, we use three cameras - one up high on a jib for the overall view, one on a tripod, manned by my wife and one shoulder mounted which I operate, so it's difficult for photogs to block all three shots.

I've only had two occasions where photogs weren't reasonable and willing to work with us.  Both times, things got a little rocky and a bit physical, but photogs went home (mad) early and we stayed and got the shots.
